The Bell Tolls for the Relationship between House Dust Mite Exposure and Asthma in Childhood

چکیده
WINNING ABSTRACT: Prior evidence has suggested a direct relationship between the level of house dust mite (HDM) allergen exposure, sensitisation and asthma. A recent cross-sectional study indicated a bell-shaped relationship between HDM exposure and sensitisation [1]. We examined the effects of cumulative HDM allergen exposure during the first 5 years of life on outcomes at age 5 years in Sydney, a high HDM environment. In the Childhood Asthma Prevention Study, bed dust samples collected at 3, 6, 9 and 12 months and then 6-monthly (total 6615 samples) until age 5 years were analysed for HDM allergen by ELISA. At five years, 516 children were assessed for wheeze and asthma by questionnaires and spirometry, and 488 for sensitisation by skin test. Time-weighted average HDM allergen concentration was estimated for each individual. The range was divided into quintiles with cut-points being 3,5; 7,8; 13,5 and 23,4 ug Der p 1/g. The association between HDM levels and outcomes was analysed with chi-square test, adjusting for confounders. The figure shows the prevalence of outcomes by quintile of time-weighted average of HDM allergen. In this birth cohort, there is a bell-shaped dose response between HDM levels and sensitisation and asthma. This may reflect tolerance to HDM allergen or be attributable to protection from associated exposures, and prompts further research on the mechanisms.

Correlation of total serum IgE and density of house dust mites with severity of Atopic Dermatitis
Background and aim: Atopic dermatitis (AD) is a chronic relapsing skin disease that commonly begins during infancy and early childhood. Research studies refers to a relationship between exacerbation of atopic dermatitis and exposure to house dust mite (HDM), but the role of HDM in the pathogenesis of AD is controversial. The objecitve of this study was to assess the correlation of density...

متن کاملHouse dust mite barrier bedding for childhood asthma: randomised placebo controlled trial in primary care [ISRCTN63308372]
BACKGROUND The house dust mite is the most important environmental allergen implicated in the aetiology of childhood asthma in the UK. Dust mite barrier bedding is relatively inexpensive, convenient to use, and of proven effectiveness in reducing mattress house dust mite load, but no studies have evaluated its clinical effectiveness in the control of childhood asthma when dispensed in primary c...
متن کاملHouse dust mite sensitivity in childhood asthma.
The clinical features of perennial asthmatic children with a skin or bronchial reaction to the house dust mite (Dermatophagoides pteronyssinus) were compared with those of asthmatic children without mite sensitivity. Mite sensitive asthma was characterised by an early age of onset of symptoms, these being predominantly nocturnal. A history of wheezing precipitated by dust exposure, during vacuu...
متن کاملHouse dust mite exposure as a risk factor for asthma: benefits of avoidance.
There is a dose-response relation between the level of exposure to house dust mite allergen and the risk of acquiring sensitization to this allergen as well as the risk of developing asthma. This relation is apparent at levels of exposure below 10 microg/g. In communities where nearly all houses have house dust mite allergen levels above this threshold, all susceptible individuals will be affec...
